To join the wine club, please read and accept the agreements below

	I am over 21 years of age.
	I accept a charge to my credit card by Jarvis to not exceed $400 per wine value three times annually.
	I accept membership into the Jarvis Wine Club and acknowledge that membership may be cancelled at any time after the first year anniversary with a written request received one month prior to next shipment.
